Cancellation Policy
24-hour notice is required to cancel or reschedule an appointment.

Failing to comply with this office policy will result in the following charges:
$35 (med refill/follow-up)
$45 (physical/surgery)
*After years of multiple daily no shows and/or same day cancellations,
we unfortunately had to implement this policy.
We thank you in advance for your cooperation and understanding.
Annual Physical Exams
Our office requires our patients to receive a
physical examination with labs each year. Labs must be completed no later than 3 business days before the physical appointment. Failure to comply will result in having to cancel the appointment with a charge of $45.
Medication
We do not offer pain management with medication, nor do we prescribe
certain medications such as Xanax, Ambien, or Immediate Release Adderall.
However, we can treat all of your primary care needs including semi-urgent
care, and refer you to specialty offices that can.
New Patient Policy
-Your first visit with be with a Mid-level provider, one of
Dr. Baggett's assistants, a NP or PA.
-You will need a Yearly Physical with labs within 30 days of your first visit,
unless you have a copy of a recent one within the year.
Refunds
We do not offer refunds after any visits.
SPC providers will develop the most appropriate and up to date treatment plans based on patient's current disease process.
Treatment plans that do not include expected medications and/or treatments do not warrant a refund.
We seek to follow the newest guidelines and accordingly, we do not recommend long-term pain pill use as well as use of Xanax or immediate release Adderall.
We will work with you to help you in ways that are safe for you that include other medications that have been proven to be effective with less potential for adverse effects.
In addition, we highly recommend everyone keep any/all controlled substances safely locked away and out of reach of others.
To dispose of any unused medication(s), specifically controlled substances,
there are two Walgreens in our area that provide medication disposal drop offs;
